Older Adult Community Mental Health Nurse/Practitioner

Dorset HealthCare University NHS Foundation Trust is seeking a highly skilled and dedicated Community Mental Health Practitioner to join our team in Weymouth. As a key member of our Community Mental Health Team for Older People, you will play a vital role in providing high-quality care and support to individuals suffering from severe mental illness.
About the RoleThis is an exciting opportunity to work in a dynamic and forward-thinking team, providing assessment, treatment, and support to older adults with complex mental health needs. As a Community Mental Health Practitioner, you will work closely with our multidisciplinary team, including psychiatrists, psychologists, nursing staff, and support workers, to deliver person-centered care and promote recovery.
Key Responsibilities- Assess, plan, implement, and evaluate programs of care for older adults with complex mental health needs.
- Conduct risk assessments and develop effective risk management plans.
- Work collaboratively with the multidisciplinary team to deliver high-quality care and support.
- Develop and maintain strong relationships with patients, carers, and other professionals to ensure seamless care coordination.
- Participate in the development and implementation of care plans, ensuring they are tailored to the individual's needs and goals.
To be successful in this role, you will need:
- Registration with the Nursing and Midwifery Council (NMC) as a Registered Mental Nurse (RMN).
- Experience working with older adults with complex mental health needs, preferably in a community setting.
- Excellent assessment, planning, and implementation skills, with the ability to prioritize and manage complex cases.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, carers, and other professionals.
- Ability to work autonomously and as part of a team, with a flexible approach to working hours and duties.
