About the Role

The Lewisham Care Partnership is seeking a highly skilled and motivated individual to join our team as a Social Prescriber. As a key member of our team, you will play a vital role in supporting the delivery of our Comprehensive Model of Personalised Care.

Key Responsibilities

  • Develop trusting relationships with individuals, their families, and carers to provide person-centred support and empower them to take control of their wellbeing.
  • Work collaboratively with local agencies, including VCSE organisations and community groups, to reduce health inequalities and promote community resilience.
  • Provide education and guidance to non-clinical and clinical staff on community services and support available to patients.
  • Manage and prioritise a caseload, ensuring timely and effective support to individuals.
  • Contribute to the development and implementation of community-based initiatives and projects.
  • Participate in regular supervision sessions, training, and development activities to enhance professional knowledge and skills.

Requirements

  • Knowledge of the personalised care approach and its application in community settings.
  • Understanding of the wider determinants of health and their impact on communities and individuals.
  • Experience of working in a community development context, adult health and social care, or public health/health improvement.
  • Ability to communicate effectively with diverse groups and individuals, including those with mental health needs.
  • Strong organisational and time management skills, with the ability to work under pressure and meet deadlines.
  • Commitment to equality, diversity, and inclusion, with a focus on reducing health inequalities.
