About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at City and County Healthcare Group Ltd. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with complex needs, including acquired brain injuries, catheter care, and advocacy.

Key Responsibilities
  • Provide person-centered care and support to individuals in their homes and in the community
  • Assist with daily living tasks, such as personal care, household chores, and meal preparation
  • Support individuals to engage in activities and hobbies that promote their physical and emotional well-being
  • Work collaboratively with other healthcare professionals to develop and implement individualized care plans
  • Provide advocacy and support to individuals to ensure their rights and needs are met
