Job Title: Senior Care Coordinator/Care Manager

Job Summary:

The North East & North Cumbria Integrated Care Board is seeking a highly skilled and experienced Senior Care Coordinator/Care Manager to join our team. As a key member of our organization, you will play a vital role in delivering high-quality care services to our communities.

Main Responsibilities:

  • Provide clinical expertise to support the delivery of complex care within Sunderland and South Tyneside.
  • Take a lead clinical role for complex care, ensuring that inter-dependencies with Secondary Care, Mental Health, Maternity, Community Services, Urgent Care, Ageing Well, and Children and Young People's transformation programs are understood and meet the needs of the local populations.
  • Manage the identification of individual and service improvement opportunities and develop service and pathway redesign projects aligned to the delivery plan.
  • Travel within North East and North Cumbria as required.

Requirements:

  • Professional Qualification (NMC, HCPC, or equivalent)
  • Evidence of sustained personal and professional development
  • Degree-level qualification or equivalent
  • Master's-level qualification or equivalent experience
  • Significant healthcare experience with a demonstrable track record of success in NHS organizations
  • Proven track record of implementing and overseeing improvements in performance and standards of care and delivering change within an organization
  • Proven track record of transformation, planning, and delivery across a range of areas and services with specific insight into commissioning
  • Experience in the management of multi-project programs of work
  • Experience in developing program or project plans across multiple partners and stakeholders, achieving consensus and managing successful delivery
  • Experience in performance management approaches, including analysis of data and performance reporting
  • Knowledge and experience of working within financial budgets
  • Whole systems management
  • Experience of commissioning and oversight and assurance/regulatory organizations
