Job Summary:
The North East & North Cumbria Integrated Care Board is seeking a highly skilled and experienced Senior Care Coordinator/Care Manager to join our team. As a key member of our organization, you will play a vital role in delivering high-quality care services to our communities.
Main Responsibilities:
- Provide clinical expertise to support the delivery of complex care within Sunderland and South Tyneside.
- Take a lead clinical role for complex care, ensuring that inter-dependencies with Secondary Care, Mental Health, Maternity, Community Services, Urgent Care, Ageing Well, and Children and Young People's transformation programs are understood and meet the needs of the local populations.
- Manage multi-project programs of work, developing program or project plans across multiple partners and stakeholders, achieving consensus, and managing successful delivery.
- Experience in performance management approaches, including analysis of data and performance reporting, is essential.
- Knowledge and experience of working within financial budgets and whole systems management are also required.
Requirements:
- Professional Qualification NMC, HCPC, or equivalent.
- Evidence of sustained personal and professional development.
- Degree-level qualification or equivalent.
- Master's level qualification or equivalent experience.
- Significant healthcare experience is essential, with a demonstrable track record of success in NHS organizations.
