Job Title: Senior Care Coordinator/Care Manager

The North East & North Cumbria Integrated Care Board is seeking a highly skilled and experienced Senior Care Coordinator/Care Manager to join their team. This is a challenging and rewarding role that requires a strong clinical background and excellent leadership skills.

Main Responsibilities:
  • Provide clinical expertise to support the delivery of complex care within Sunderland and South Tyneside.
  • Take a lead clinical role for complex care, ensuring that inter-dependencies with Secondary Care, Mental Health, Maternity, Community Services, Urgent Care, Ageing Well and Children and Young Peoples transformation programmes are understood.
  • Travel within North East and North Cumbria to meet the needs of the local populations.
Requirements:
  • Professional Qualification NMC, HCPC or equivalent.
  • Evidence of sustained personal & professional development.
  • Degree level qualification or equivalent.
  • Master's level qualification or equivalent experience.
Key Skills:
  • Programme management skills and experience.
