Job Summary
We are seeking a highly skilled and experienced CHC Senior Practitioner to join our Continuing Health Care Team in Greater Manchester. As a key member of our team, you will play a vital role in supporting the assessment and review process for NHS Continuing Care Framework.
Main Responsibilities
As a CHC Senior Practitioner, you will be responsible for:
- Coordinating the process of determination of eligibility for NHS funded Continuing Care within the parameters of the National Framework for Children and Young People's Continuing Care (2016)
- Deputising for the Lead Nurse in issues around Continuing Healthcare, NHS Funded Care, and Complex Care
- Representing NHS GM Integrated Care at multi-agency meetings and liaising with other professionals, providers, and key stakeholders as required
- Liaising with patients, families, and carers regarding the National Framework for Children and Young People's Continuing Care and review process
- Managing a caseload of patients who have been assessed as eligible for NHS Continuing Care or a jointly funded package of individually commissioned support

Person Specification
We are looking for a candidate with:
- Registered Nurse with current NMC registration
- Diploma in Health Education or BSC (Hons) Degree in Nursing or related subjects
- Expert knowledge/understanding of current national and local policy/Framework Guidance relating to NHS Continuing Care
- Experience of working as a Registered Nurse with patients with a Learning Disability or Autism
- Experience of clinical supervision and reflective practice discussions
- The ability to lead multi-professional MDTs
