Job Summary:

Greenbank Surgery seeks an experienced Practice Nurse to join our dynamic team of clinical and non-clinical staff. The ideal candidate will possess excellent knowledge of Primary Care and provide high-quality nursing services to our patient population.

Key Responsibilities:

  • Work alongside the practice nursing team to deliver effective nursing services.
  • Ensure nursing services are delivered effectively to the entitled patient population.
  • Responsible for clinical areas such as infection prevention and control, health promotion, chronic disease management, and more.

INDUCTION TRAINING:

A comprehensive induction program will be provided to all new personnel, ensuring a smooth transition into the practice.

Effective communication and teamwork are essential in our multidisciplinary environment. Staff are expected to recognize the significance of collaborative working and communicate in a manner that enables the sharing of information.

Service Delivery:

Staff at Greenbank Surgery must adhere to the information contained within practice policies and regional directives, ensuring protocols are followed at all times.

SECURITY:

The security of the practice is the responsibility of all personnel. Staff must remain vigilant and report any suspicious activity to their line manager.

PRIMARY RESPONSIBILITIES:

  • Act as the Infection Prevention Control (IPC) lead and work to the IPC Policy.
  • Develop, implement, and embed health promotion and well-being programs.
  • Implement and evaluate individual treatment plans for chronic disease patients.
  • Provide routine nursing care to patients as required.
  • Diagnose and manage acute and chronic conditions.
  • Prescribe medication for patients in accordance with PGDs and own scope of practice.
  • Review patient medications to enhance compliance.
  • Provide guidance to patients in the use of prescribed and over-the-counter medications.
  • Oversee the provision of wound care services to patients.
  • Co-ordinate the provision of travel medicine services.
  • Request pathology services as necessary.
  • Process pathology results as required.
  • Maintain accurate clinical records in conjunction with extant legislation.
  • Ensure read codes are used effectively by all members of the nursing team.
  • Ensure the appropriate maintenance and use of chronic disease registers.
  • Develop, implement, embed, and participate in well woman and well man clinics.
  • Chaperone patients where necessary.
  • Assist GPs with minor surgery when required.
  • Prioritise health issues and intervene appropriately.
  • Support the team in dealing with clinical emergencies.
  • Recognise, assess, and refer patients presenting with mental health needs.
  • Implement vaccination programs for adults and children.
  • Support patients in the use of their prescribed medicines or over-the-counter medicines.
  • Liaise with external services/agencies to ensure patient support.
  • Delegate clinical responsibilities appropriately.
  • Support the clinical team with safeguarding matters.
  • Understand practice and local policies for substance abuse and addictive behaviour.
  • Deliver opportunistic health promotion where appropriate.
