About the Role

Folly Lane Medical Centre is seeking a highly skilled and motivated Practice Nurse to join our team. As an autonomous practitioner, you will be responsible for delivering high-quality patient care and working collaboratively with our general practice team to meet the needs of our diverse patient population.

Main Responsibilities

  • Assess, plan, develop, support, implement and evaluate treatment programmes that promote health and well-being and apply principles of self-care.
  • Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
  • Work with other healthcare professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.
  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
  • Work with patients in order to support adherence to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support and manage health needs of women presenting for family planning and cervical cytology consultations.
  • Recognise, assess and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
  • Implement and participate in vaccination and immunisation programmes for both adults and children.
  • Advise, support and administer vaccinations where appropriate for patients travelling abroad.
  • Promote and deliver evidence-based care for patients presenting with aural conditions.
  • Meet the needs of patients presenting for opportunistic wound care.
  • Assessing problems presented opportunistically by patients, dealing with minor illness.
  • Able to document Consultations according to NMC guidelines.
  • Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.
  • Perform a holistic assessment of patients attending for cervical cytology smear tests.
  • Perform venepuncture according to local guidelines.
  • Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines.
  • Take ECGs.
  • Able to recognise and manage anaphylaxis according to current UK guidelines.
  • Able to perform Cardio-pulmonary resuscitation according to current UK guidelines.
  • Ensure infection control guidelines are maintained.
  • Ability to monitor and manage maintenance of stock and equipment to include refrigeration, steriliser and emergency equipment.
  • Confidentially of information gained at work must be preserved at all times.
  • Cover the assistance in the provision of minor surgery when other colleagues not in practice.

Folly Lane Medical Centre is a five-partner forward-thinking training practice with a practice list size of over 10,200 patients. We are a friendly and supportive team with an excellent working environment. We are otherwise fully staffed. Our clinical team includes:

  • 5 GP partners
  • 3 Salaried GPs
  • GP trainees
  • Nursing team: Nurse Practitioner, Assistant Practitioner, Practice Nurse, HCA.
  • Reception. Administration Team
  • Mental health practitioner
  • With additional ARRS staff including Clinical Pharmacist, Pharmacy Technician, FCP, Care Co-ordinator, GP Assistant, Social Prescriber and Health & Wellbeing Coach.
  • We have good relationships with other practices and play an active role in our PCN, with one of our partners being the Clinical Director.
