About Folly Lane Medical Centre

Folly Lane Medical Centre is a friendly and proactive practice located in the centre of Warrington, serving a diverse population. We have a strong practice team who work hard to deliver excellent patient care. We are seeking a skilled Practice Nurse to join our team, working 20 hours per week, over a Monday, Wednesday, and Friday. The successful candidate will be responsible for delivering general practice to our whole practice population, focusing on the delivery of evidence-based practice for patients with long-term conditions and the provision of preventative healthcare.

Main Responsibilities

  • Assess, plan, develop, support, implement, and evaluate treatment programmes that promote health and well-being.
  • Assess, plan, implement, and evaluate individual treatment plans for patients with known long-term conditions.
  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition.
  • Work with other healthcare professionals to diagnose, monitor, manage, and treat long-term conditions, including non-drug-based treatment methods.
  • Review medication for therapeutic effectiveness and provide information and advice on prescribed or over-the-counter medication.
  • Prioritise health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
  • Support and manage health needs of women presenting for family planning and cervical cytology consultations.
  • Recognise, assess, and refer patients presenting with mental health needs.
  • Implement and participate in vaccination and immunisation programmes for both adults and children.
  • Advise, support, and administer vaccinations where appropriate for patients travelling abroad.
  • Promote and deliver evidence-based care for patients presenting with aural conditions.
  • Meet the needs of patients presenting for opportunistic wound care.
  • Able to document consultations according to NMC guidelines.
  • Ensure awareness of statutory and local clinical protection procedures, including systems of referral.

Person Specification

  • Essential:
    • Education: English and Math GCSE grades A and C and higher level qualifications.
    • Qualifications: Registered general nurse qualifications/certificates.
    • Experience in Primary Care/GP.
    • Understanding the principles of audit.
    • Knowledge of IT clinical systems (SystemOne).
    • Chronic disease management.
    • Commitment to professional development.
  • Desirable:
    • Knowledge of Microsoft Office software.

We are a five-partner forward-thinking training practice with a practice list size of over 10,200 patients. We are a friendly and supportive team with an excellent working environment. We are otherwise fully staffed. Our clinical team includes:

  • 5 GP partners.
  • 3 Salaried GPs.
  • GP trainees.
  • Nursing team: Nurse Practitioner, Assistant Practitioner, Practice Nurse, HCA.
  • Reception and Administration Team.
  • Mental health practitioner.
  • With additional ARRS staff including Clinical Pharmacist, Pharmacy Technician, FCP, Care Co-ordinator, GP Assistant, Social Prescriber, and Health & Wellbeing Coach.

We have good relationships with other practices and play an active role in our PCN, with one of our partners being the Clinical Director.
