Annual Salary: £32,000 - £40,000

Your primary responsibility will be to assess, manage, and review individuals' needs and risks, focusing on prevention and protection of vulnerable adults. You will develop and implement care plans that promote independence and community integration, while collaborating with multidisciplinary teams to ensure comprehensive support.

Main Accountabilities:

  • Conducting thorough assessments of individuals' needs and risks to inform care planning.
  • Collaborating with healthcare professionals to ensure seamless transitions between services.
  • Maintaining accurate records of assessments, interventions, and outcomes to inform future decisions.

Key Requirements:

  1. A recognised Social Work Qualification, preferably in mental health, demonstrating expertise in adult safeguarding.
  2. Current registration with Social Work England, evidencing commitment to ongoing professional development.
  3. A valid DBS Check (within 12 months or Update Service registered), verifying suitability for work with vulnerable populations.
