Patient Advocacy Specialist

Key Responsibilities:
- Conduct thorough assessments of patient health, identifying areas for improvement and developing targeted interventions.
- Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans, aligning with patient values and goals.
- Provide education and support to patients and their families, empowering informed decision-making and healthy outcomes.
- Maintain accurate and up-to-date patient records, ensuring compliance with relevant healthcare regulations.
Requirements:
- A Registered Nurse (RN) license in good standing with the relevant nursing board.
- Previous experience in a clinical or hospital setting, demonstrating strong clinical skills and judgment.
- Excellent communication and interpersonal skills, enabling effective collaboration with patients, families, and healthcare teams.
- The ability to work in a fast-paced and dynamic environment, adapting to changing circumstances and priorities.
