Job Title: Community Psychiatric Nurse/Mental Health Practitioner

Nottinghamshire Healthcare NHS Foundation Trust is currently seeking a highly skilled and experienced Community Psychiatric Nurse/Mental Health Practitioner to join our team.

About the Role:
  1. Provide high-quality, patient-centered care to adults living in the community, promoting recovery and well-being.
  2. Work collaboratively with multidisciplinary teams to develop and implement comprehensive care plans, addressing complex mental health needs.
  3. Conduct thorough assessments, interventions, and evaluations to ensure optimal patient outcomes.
  4. Support patients and families in managing mental health conditions, offering guidance and resources.
  5. Participate in education and training initiatives to enhance clinical knowledge and skills.
Key Responsibilities:
  1. Caseload management for patients living in the community.
  2. Act as a care coordinator, overseeing referrals, initial assessments, and one-to-one interventions.
  3. Develop and implement treatment plans, utilizing specialist skills and knowledge.
  4. Establish and maintain robust communication networks with clients, carers, and healthcare professionals.
  5. Use interpersonal skills to diffuse potentially emotive situations during assessments and pathway planning.
Person Specification:
  1. Professional Qualification (Mental Health) to degree level or equivalent.
  2. Maintains current registration with the NMC.
  3. Further specialist knowledge or post-registration experience in specific service areas or formal training.
Preferred Qualifications:
  1. Demonstrates continued professional development.
