About the Role

We are seeking a dedicated and compassionate Senior Care Support Worker to join our team at Mary Ann Evans Hospice. As a key member of our community services, you will play a vital role in providing high-quality palliative and end-of-life care to patients and their families in their own homes.

Key Responsibilities
  • Provide expert care and support to patients and their families in their own homes, promoting independence and dignity.
  • Work closely with our community and hospital colleagues to ensure seamless care coordination and effective communication.
  • Develop and maintain strong relationships with patients, families, and carers, ensuring their needs are met and their voices are heard.
  • Contribute to the development and implementation of care plans, ensuring they are tailored to individual needs and preferences.
  • Participate in ongoing training and professional development to enhance skills and knowledge in palliative and end-of-life care.
Requirements
  • Level 3 Diploma/NVQ Level 3 in Health and Social Care or equivalent.
  • Minimum 1 year's experience in care, preferably in palliative and end-of-life care.
  • Excellent communication and interpersonal skills, with the ability to work autonomously and as part of a team.
  • Experience of supervising and supporting team members, with a commitment to ongoing professional development.
