Community Health Nurse

We are seeking a skilled and compassionate Community Nurse to join our team in providing high-quality healthcare to patients in their homes and local clinics.
Key Responsibilities- Deliver skilled nursing care to patients in a variety of community settings, working under the supervision of the Community Nurse Team Lead and Deputy Team Lead.
- Undertake comprehensive assessments of patients' physical, psychological, and social care needs, using the Single Assessment Process.
- Develop and implement individualized care plans that promote self-care, independence, and seamless nursing care.
- Act as a Named Nurse, taking continued responsibility for the holistic health needs assessment, care planning, evaluation, and implementation of healthcare.
- Collaborate with multidisciplinary teams to coordinate patient care, promoting effective communication and coordination.
- Registered Nurse (RN) with NMC Registration.
- Post-registration experience in an NHS community environment.
- Recent clinical nursing experience negotiating and liaising within a multidisciplinary team.
- Knowledge of out-of-hospital care and the role of the Community Nurse.
- Awareness of the management and use of data in the NHS setting.
