- Provide statutory Adult Social Care and Care Management functions, including assessment, care planning, and reviewing.
- Liaise with statutory and non-statutory agencies to ensure effective service delivery.

Requirements- BSC, BA, MA, or equivalent in Social Work, with registration with Social Work England.
- Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff.
- Experience of working within a multi-disciplinary team, with a focus on community structures and social care management responsibilities.
- Knowledge and understanding of recovery principles in Mental Health and strengths-based Social Work Practice.
