Job Description
We are seeking an experienced Children and Young People's Community Nurse to join our dynamic team, providing high-quality care to children and young people with short-term and long-term conditions, including those requiring continuing care packages. As a key member of our community nursing service, you will work flexibly to deliver evidence-based nursing care, prevent hospital admissions, and facilitate early discharge. Your role will involve collaborating with multi-agency partners, promoting effective communication, and meeting the complex health needs of identified children through liaison with schools and other stakeholders.

Main Responsibilities
  • Deliver high-quality, patient-centered care to children and young people with diverse needs, focusing on preventative measures and early intervention strategies.
  • Liaise closely with school staff and other healthcare professionals to ensure seamless care coordination and collaborative goal-setting.
  • Promote safe and efficient transfer of patients between settings, adhering to established protocols and guidelines.
  • Contribute to the development of training programs for colleagues working with families, focusing on shared learning opportunities and enhanced support strategies.
  • Collaborate with consultants and other specialists to enhance medical outpatient clinic services within designated geographical areas.

Required Skills and Qualifications
  • Nursing degree-level education or equivalent qualification.
  • Minimum two years' experience in children's community nursing, preferably in a leadership or specialist capacity.
  • Demonstrable expertise in areas such as long-term ventilation, IV therapy, TPN, caseload management, safeguarding children, teaching ventilation, and non-medical prescribing.
  • Proven ability to work independently, make sound judgments, and prioritize tasks effectively in fast-paced environments.
  • Excellent interpersonal and communication skills, with a commitment to patient-centered care and continuous quality improvement.
