We are seeking a skilled and compassionate Social Worker to join our team in providing specialized care services for children with disabilities. This is an exciting opportunity to make a meaningful difference in the lives of individuals and families.
About the Role:
- This position involves working closely with families, healthcare professionals, and other agencies to develop and implement individualized care plans that cater to the unique needs of each child.
- You will be responsible for assessing the needs of children and adolescents with severe or significant challenges, including learning disabilities, physical limitations, sensory impairments, or profound communication barriers.
- Your role will involve responding to new referrals promptly and ensuring effective initial interventions while nurturing enduring relationships with families to support their aspirations.

Requirements:
- A social work qualification and registration with Social Work England.
