Salary and Benefits

The successful candidate will receive an annual salary of approximately £18,668, equivalent to £14.36 per hour, based on a pro rata 20-hour workweek.

About the Role

We are seeking an experienced Support Coordinator to join our team in coordinating and providing person-centred support for our Finding and Matching Service. This role involves working closely with the Outreach Manager and Senior Project Worker to achieve service users' goals, promoting their participation in community activities and facilitating decision-making processes.

Duties and Responsibilities

  • To administer case work for assigned service users, handling various issues and concerns while maintaining effective communication with family members, support workers, and professionals.
  • To empower service users in managing their needs, including health services, personal care, finances, and benefits, while adhering to safeguarding procedures and reporting any signs of abuse or concern.
  • To collaborate with Care Managers, families, advocates, DWP, and other stakeholders to provide comprehensive support and ensure the service's compliance with Health and Safety standards.
  • To maintain accurate records, write reports, and utilise IT systems such as Microsoft Office and Charity Log data systems.

Required Skills and Qualifications

Candidates should possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment. A background in social care or a related field is essential, along with experience in administering case work and collaborating with various stakeholders.
