Job Summary

We are seeking a highly skilled and motivated Clinical Support Coordinator to join our team at the Central London Community Health Trust. The successful candidate will be responsible for coordinating and driving the delivery of services for High Intensity Users (HIUs) of Urgent and Emergency Care services.

About the Role

As a Clinical Support Coordinator, you will be responsible for identifying HIUs who require additional support, reviewing their needs, and referring them to suitable providers. You will also be responsible for tracking processes and outcomes, providing information and training to local health and social care professionals, and working with a wide range of health, social care, and third sector organizations to prioritize care for this highly vulnerable group.

Main Duties

* Identify an agreed number of HIUs attending ED each month
* Review their needs with suitable clinical and other support
* Identify which services are best placed to provide that support
* Refer patients and ensure their care is picked up
* Track processes and outcomes of the service
* Provide information and training to local health and social care professionals

Requirements

* Excellent IT skills and a positive attitude towards learning and using your own initiative for problem-solving
* Proven record of excellent written and verbal communication and interpersonal skills
* Good organizational and time management skills
* Able to prioritize and manage own workload and show initiative to complete tasks independently and with multi-disciplinary teams
* Excellent motivational and influencing skills
* Proficient in Microsoft applications and familiar with IT systems
