About the Role

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Home Group Limited. As a Support Coordinator, you will play a vital role in supporting our customers with complex mental health needs, learning difficulties, and/or dual diagnosis.

Key Responsibilities
  • Assist in the delivery of tasks based on customers' personal support plans, needs, and aspirations.
  • Work closely with external agencies and professionals to assess and engage support for our customers.
  • Empower customers to develop independent living skills, enabling them to successfully move on.
  • Collaborate with colleagues to ensure the smooth delivery of support.
  • Carry out risk assessments, support planning, goal setting, and regular reviews.
Requirements
  • Experience in safeguarding, risk assessments, confidentiality, support plans, and collaborative working.
  • Ability to work on your own initiative, positively influencing people from diverse backgrounds.
  • Previous experience in a similar environment, with a focus on supporting vulnerable individuals.
  • Valid driving license and access to a vehicle insured for business purposes.
  • Enhanced DBS check (paid for by Home Group Limited).
