About the Role

We are seeking an experienced Clinical Coder to join our team at the Dartford and Gravesham NHS Trust. As a Clinical Coder, you will play a crucial role in ensuring the accuracy and completeness of clinical data for our patients.

Key Responsibilities
  • Abstract, analyze, and accurately code clinical information from patient records using ICD-10 and OPCS-4 Classifications.
  • Produce consistent, accurate, complete, and timely coded clinical data for use by the Trust's Finance Department and other stakeholders.
  • Engage and support the internal continuous audit process to ensure a high level of accuracy and compliance with National Standards and local policies.
  • Assist in the daily supervision of trainee and less experienced clinical coding staff, providing mentorship and cultivating a supportive learning environment.
  • Interpret histology and radiology reports to ensure accuracy of the coding process.
  • Undertake the coding of extracted information, ensuring adherence to National Standards and local policies.
  • Accurately enter clinical codes onto the Trust's information system, PAS/Medicode.
  • Identify insufficient or incorrect clinical data in source documentation and raise queries for resolution with other relevant departments.
  • Share and assist colleagues with personal coding experience, promoting a learning environment at the workplace.
  • Organize daily workload efficiently to meet personal coding targets, including allocated Consultant backlog list, supporting the coding team in achieving high standards of coding completeness and accuracy at agreed timescales.
  • Take responsibility for self-development on a continuous basis by studying for the Auditor and/or Trainer qualification, if required by the Trust.
  • Participate in ongoing coding training and testing to meet National Standards and Trust requirements.
  • Liaise with Clinicians and their support staff concerning interpretation of case note documentation affecting coding.
  • Investigate and correct coded clinical data rejected or queried by recipient information systems.
  • Communicate complex coding rules to clinical coding staff, medical, clinical, and administrative staff, and external agencies.
  • Implement agreed local policy directives and assist in the development of local coding policies that impact on various service areas.
  • Undertake/participate in complex audits.
  • Work across all specialties.
Person Specification

Education and Training

  • Accredited Clinical Coder qualification.
  • Coding Standards Course.
  • Recent Refresher workshop.
  • Evidence of ongoing professional development.

Knowledge

  • Knowledge of medical terminology, anatomical, and physiological terms acquired through work-based training.
  • Good knowledge and use of information/informatics systems used within the NHS.
  • Knowledge and participation in service development projects.

Experience

  • Recent clinical coding experience in an Acute NHS Trust.
  • Experience of working within the NHS.
  • Excellent command of English for all personal, written, and electronic forms of communication.
  • Proven ability to liaise with multidisciplinary teams at all levels of NHS Acute Trusts structure.

Desirable

  • Use of Patient Centre PAS.
  • Medicode.
