Key Responsibilities

The successful candidate will be tasked with extracting, analyzing, and accurately coding clinical data from patient records (both paper and electronic) across various specialties, utilizing ICD-10 and OPCS-4 classifications in alignment with National Standards and the Trust's Policies and Procedures. The role demands the production of consistent, precise, complete, and timely coded clinical data for use by the Trust's Finance Department and other stakeholders, all while adhering to increasingly stringent deadlines without compromising accuracy.

Our coding documentation comprises a blend of electronic and some paper records, with a strong emphasis on transitioning to a fully electronic patient record system.

Required Qualifications

  • Possession of the National Clinical Coding Qualification (NCCQ) UK or actively pursuing it
  • Experience with ICD-10 and OPCS-4 classifications (current version) in an acute healthcare setting
  • Extensive understanding of medical terminology, anatomy, and physiology

Job Duties

  • Ensure precise extraction of intricate clinical details related to diagnoses and procedures for each consultant episode from patient medical records
  • Engage in and support the internal continuous audit process, achieving and maintaining a personal IG level 2 audit result
  • Assist in supervising trainee and less experienced clinical coding staff, fostering a supportive learning atmosphere
  • Interpret histology and radiology reports to guarantee the accuracy of the coding process
  • Undertake the coding of extracted information, ensuring compliance with National Standards and local policies
  • Accurately input clinical codes into the Trust's information system PAS/Medicode
  • Identify insufficient or incorrect clinical data in source documentation and proactively raise queries for resolution with relevant departments
  • Share personal coding experiences with colleagues and promote a learning environment within the workplace
  • Organize daily workload efficiently to meet personal coding targets, including the allocated Consultant backlog list, supporting the coding team in achieving high standards of coding completeness and accuracy within agreed timelines
  • Take responsibility for continuous self-development by pursuing the Auditor and/or Trainer qualification, if required by the Trust
  • Participate in ongoing coding training and assessments to meet National Standards and Trust requirements, which may involve travel to other organizations outside core working hours
  • Liaise with Clinicians and their support staff regarding the interpretation of case note documentation affecting coding
  • Investigate and rectify coded clinical data rejected or queried by recipient information systems
  • Communicate complex coding rules to clinical coding staff, medical, clinical, and administrative personnel, as well as external agencies
  • Implement agreed local policy directives and assist in developing local coding policies impacting various service areas
  • Undertake or participate in complex audits
  • Work across all specialties

Education and Training Requirements

  • Recent clinical coding experience in an Acute NHS Trust
  • Experience working within the NHS
  • Excellent command of English for all forms of personal, written, and electronic communication
  • Proven ability to collaborate with multidisciplinary teams at all levels within NHS Acute Trusts

Desirable Experience

  • Familiarity with Patient Centre PAS
  • Experience with Medicode
