Job Title: Clinical Case Manager

We are seeking a highly skilled and experienced Clinical Case Manager to join our team. As a Clinical Case Manager, you will work with individuals who have suffered catastrophic injuries to assess their needs, develop personalized rehabilitation plans, and coordinate care and support services.

Key Responsibilities:
  • Assess client needs and develop rehabilitation plans
  • Coordinate care and support services, including therapy, rehabilitation, and financial support
  • Monitor progress and respond to changing needs
  • Work collaboratively with clients, families, and healthcare professionals to achieve optimal outcomes
Requirements:
  • At least 3 years of clinical experience
  • Valid clinical registration with an appropriate clinical body
  • Previous experience in case management or community settings
  • Strong relationship-building skills and ability to work independently
  • Driver with access to own vehicle and willingness to travel extensively
