About the Role

As a Clinical Case Manager, you will play a vital role in supporting our clients through their rehabilitation journey. Your expertise will be instrumental in assessing their needs, developing personalized rehabilitation plans, and advocating for their rights.

Key Responsibilities
  • Manage a caseload of clients, conducting assessments and developing rehabilitation plans
  • Lead on assessment and goal setting, ensuring clients receive the best possible support
  • Signpost and advocate for clients, connecting them with relevant services and resources
  • Manage administrative tasks, including report writing and communication with third parties

Requirements
