The Role

We are seeking a skilled and experienced Clinical Case Manager to join our team. As a Clinical Case Manager, you will be responsible for managing a caseload of clients, leading on assessments and goal setting, and signposting and advocating for the needs of our clients.

Key Responsibilities

  • Manage a caseload of clients through their rehabilitation
  • Lead on assessments and goal setting
  • Signpost and advocate for the needs of clients
  • Manage all administration related to cases

Requirements

  • At least 3 years of clinical experience
  • A valid clinical registration with an appropriate clinical body
  • Previous experience in case management or working in the community setting
  • Strong relationship building skills
  • Driver with access to own vehicle
  • Willingness to travel extensively across the UK, Scotland, and Wales
