Job Title: Clinical Case Manager

We are seeking an experienced Clinical Case Manager to join our team at Active Care Group. As a Clinical Case Manager, you will be responsible for providing comprehensive assessments and developing personalized case management plans to support individuals with complex clinical needs.

Key Responsibilities:
  • Conduct thorough assessments of individuals' abilities and difficulties to identify goals and objectives for achieving independence in everyday activities.
  • Develop and implement case management plans that utilize goal-setting processes and documented outcome measures to ensure optimal levels of independence.
  • Monitor and review the implementation of case management plans, taking proactive steps to address any challenges or concerns.
  • Collaborate with clients, informal/formal carers, and other relevant professionals to discuss options for meeting needs and identifying goals for intervention.
  • Document all outcomes in accordance with our case management processes and prepare monthly summaries detailing progress and new goals agreed.
Requirements:
  • Membership of either NMC, HCPC, SWE, SWS, or SWW.
  • Minimum of 5 years post-qualifying experience in a relevant field.
  • Experience working with clients suffering from spinal cord injuries, traumatic and acquired brain injuries, and other complex conditions.
  • Ability to work in community settings and with clients of all ages.
