Palliative Care Hospital Liaison Specialist

This exciting opportunity has arisen for a Band 7 Specialist Palliative Care Nurse to join our Palliative Care Hospital Liaison Team. As part of our multidisciplinary service, you will provide expert specialist palliative care for patients with life-limiting illnesses within hospital settings.
Main Responsibilities- Develop partnerships with members of the Multidisciplinary Team to provide seamless palliative care services.
- Provide clinical professional support, resources, and education to team members as needed.
- Work collaboratively with other professionals to promote and develop the ethos of Palliative Care within the Trust and local community.
- NMC Registered Nurse
- Diploma/Degree level of study
- Registered Mentor
- Teaching qualification or equivalent
- Supplementary prescribing qualification
- Diploma in Palliative/Cancer Care
- Community nursing experience
