Adult Service Coordinator at Choice Care.
We are seeking a motivated and detail-oriented individual to join our team as an Adult Service Coordinator. This role is responsible for coordinating and delivering adult services in a residential setting, working closely with a team of care workers to provide high-quality support and care to individuals with learning disabilities, autism, and mental health conditions. As an Adult Service Coordinator, you will be part of a dynamic team that prioritizes the well-being and empowerment of our service users.

Key Responsibilities:
  • Coordinating and delivering adult services in a residential setting, including developing and implementing care plans, monitoring progress, and making recommendations for improvement.
  • Liaising with managers and other shift leaders to ensure seamless care delivery.
  • Supervising and sharing mealtimes with residents to encourage positive attitudes towards food and dining.
  • Maintaining accurate records and reports, including incident reports, risk assessments, and medication administration records.

Requirements:
This role requires a patient, reliable, and flexible individual who can work effectively in a team environment. You must have good communication skills and experience in the care sector, as well as a willingness to learn and expand your knowledge. In addition, you should hold relevant qualifications such as the Level 2 Adult Care Worker and Lead Adult Care Worker Level 3 qualifications.
