Clinical Care Specialist

About this role:
We are seeking a skilled and experienced Clinical Assessor to join our team at Hampshire and Isle of Wight Integrated Care Board (ICB). As a Clinical Assessor, you will play a vital role in supporting individuals across all ages who require ongoing care.
Key responsibilities:
- Co-ordinate and deliver needs/eligibility assessment, case management and review of patients under the relevant frameworks for NHS Continuing Healthcare, Funded Nursing Care, Fast Track.
- Work within the NHS All Age Continuing Care team to ensure consistent, equitable, safe and cost-effective case management.
- Develop, monitor and review programmes of care to meet identified needs, including the giving of specialist advice to patients, carers and members of health and social care teams.
Requirements:
- Registered Health or Social Care Professional with current registration with professional body NMC, HCPC or SWE.
- Knowledge of recent developments in the NHS.
- Working knowledge of IT systems.
- Clinical assessment skills.
