About the Role

We are seeking a highly motivated and dedicated Mental Health Recovery Support Worker to join our team at Excelcare Holdings.

Key Responsibilities

  • Provide a high-quality support service to address the needs of our residents, working closely with the Unit Team Leader and Group Manager.
  • Lead and support a caseload of allocated residents, including comprehensive personal needs and risk assessments to inform individual support plans.
  • Ensure support plans are SMART and evidence continual progress, incorporating regular reviews and monitoring.
  • Develop a support and guidance approach that is engaging, empathetic, and flexible, tailored to each individual's required goals and outcomes.
  • Work in partnership with internal and external stakeholders to support residents' needs, including external support services if required.
  • Complete onward referrals to identified move-on accommodation and support residents to achieve these goals.
  • Accompany residents to appointments if support is required, and attend meetings to share updates and outstanding issues.
  • Carry out premises inspections and participate in community activities.
  • Work alongside Support Assistants to offer coaching and guidance, while also supporting residents and the team.

About You

  • You will have experience and understanding of the issues faced by people who are homeless or vulnerably housed.
  • You will possess tenacity and a professional, positive approach to ensure continuous results are achieved.
  • You will have case holder management skills, be able to find solutions in challenging situations, and have good communication skills - written, verbal, and IT.
  • You will be able to write factual, concise, and accurate reports, and have knowledge of safeguarding principles and procedures.
  • Knowledge of current Housing legislation, welfare benefit legislation, and other regulations relevant to support and resettlement of residents is also required.
