As a key member of our team, you will work closely with families who are referred for support, taking on the lead professional role to coordinate a multi-agency whole family approach to address difficulties.

Key Responsibilities
  • Carry a complex caseload and key work with families, ensuring a high standard of service to families, including securing the services of other agencies.
  • Undertake whole family assessments, reviewing action plans regularly and coordinating activity to ensure agreed tasks are identified and goals reached in a timely manner.
  • Work in partnership with social workers to effectively step-down cases to the Early Help Service, reducing social care case loads.
  • Ensure that the service is delivered at times that suit and meet the needs of the families, including early mornings and late evenings.
Requirements
  • Significant experience of direct work with children, young people, and their families in the family home or in the community.
  • Experience of working effectively in co-operation and partnership with a range of communities, partner agencies, private sector bodies, public agencies, voluntary bodies, and statutory agencies.
  • Strong verbal and written communication skills, with the ability to work assertively to achieve sustainable and positive change, including negotiation and problem-solving in a multi-disciplinary setting.
  • Ability to keep accurate records, organise, plan, and coordinate work, and proficient use of IT systems, including Outlook, MS Office, and electronic case management systems.
