As a key member of our primary care team, you will be responsible for delivering high-quality nursing services to our patients. Your expertise will enable you to assess and manage chronic diseases, diagnose and treat a wide range of acute presentations, and provide advanced autonomous clinical skills.

Responsibilities:

- Deliver safe, high-quality primary care nursing services to the practice population.
- Work collaboratively with the multi-disciplinary general practice team and other healthcare professionals to promote integrated pathways of care.
- Provide general and specific health screenings to the practice patients within agreed protocols.
- Undertake initial assessments of patients presenting with wounds and manage independently or refer to other services as necessary.
- Participate in clinical audit and the implementation of changes to ways of working and/or clinical practice.

Requirements:

- Registered Nurse with NMC 1st level Registration.
- Evidence of continued professional development.
- Experience of working within healthcare governance systems and understands role and responsibilities for CQC and within risk management.
- Clinical skills and competence in Practice Nurse Treatment Room interventions.
- Experience of managing long-term conditions and implementing protocols and clinical guidelines.
