Job Summary:
We are seeking an experienced and skilled Practice Nurse to join our team at The Deepings Practice. As a key member of our healthcare team, you will provide high-quality nursing care to our patients, working in accordance with the NMC Code of Conduct.
The successful candidate will have a strong background in health prevention and long-term condition management, with experience in assessing, treating, and screening patients. You will work closely with our general practitioners and other healthcare professionals to provide comprehensive care to our patients.
Main Duties:
Clinical Responsibilities:
Management of Chronic Disease:
Identify significant abnormalities within Health Promotion Clinics for chronic disease, diagnose, monitor, and develop individual management plans, agreeing these as appropriate with the patient and other health professionals.
Therapeutic Monitoring:
Check compliance with and adherence to appropriate treatments using a holistic patient-centred approach, recognise abnormalities, identify the impact of treatment, and implement or recommend changes as appropriate.
Wound Care and Management:
Assess routine wounds including trauma and post-operative wounds, support good wound management in complex situations using the Doppler technique, and provide patient health checks and health screening.
Risk Assessment:
Recognise issues and gather sufficient information to refer, including drugs, domestic violence, child protection, vulnerable adults, and social problems.
Travel Health:
Provide travel risk assessments/management by administering vaccinations in accordance with guidelines/PSDs/PGDs, and provide specialist and evidence-based advice to patients.
Immunisation (Adult and Child):
Administer appropriate immunisation autonomously with PGD instructions, and work with patients with more complex immunisation issues.
Mental Health:
Communicate with key workers, administer appropriate prescribed therapies, and monitor for side effects.
Housekeeping:
Maintain cold chain, monitor stock levels, and organise equipment.
