We are seeking a highly skilled Advanced Clinical Practitioner to join our Out of Hours team at Cumbria Health. This is an exciting opportunity to work in a dynamic and fast-paced environment, providing high-quality care to patients in need.
Key Responsibilities- Work as a member of the clinical team, providing patient-centered care and making autonomous decisions to enhance the delivery of care.
- Assess, plan, and implement patient-centered plans of care, independently discharging patients from your care and referring when necessary to other healthcare professionals.
- Communicate effectively with patients, families, and other healthcare professionals, recognizing and managing situations where there are barriers to effective communication.
- Adapt to change within the working situation, managing unscheduled emergency situations and performing investigations to reach a differential diagnosis.
- Perform safe manual handling techniques and recognize own limitations, seeking assistance and referring appropriately.
- Significant post-registration experience (minimum 3 years in a relevant field, acute or primary care setting)
- Evidence of autonomous practice assessing and managing patients with undifferentiated presentations
- Leadership skills and ability to work as a support network for other colleagues
- Non-medical prescribing qualification and evidence of recent Continual Professional Development
