Key Responsibilities:

  • Develop and implement patient care plans in collaboration with healthcare professionals.
  • Assess and prioritize patient needs, providing individualized care and support.
  • Contribute to the maintenance of accurate and up-to-date patient records.
  • Participate in quality improvement initiatives to enhance patient care and services.

Flexible Working: Successful applicants can request flexible working arrangements from their first day of employment, with various provisions available depending on the role and organization.

Salary: £28,000 - £35,000 per annum, dependent on experience.
