Job Responsibilities
The successful candidate will assess patients, work with counselors, and design treatment plans for mental health needs. Develop and provide behavioral health education, conduct limited community assessments, and follow up on patient progress. The manager will serve as a liaison between patient and mental health provider, receive documentation, follow up with patients, and set up meetings as needed. Additionally, the Clinical Health Manager will provide patient education on treatment plans, resources, and confidentiality, maintain accurate patient records, and adhere to HIPAA guidelines. They will also collect and analyze data to evaluate programs and identify educational needs, and work with community contacts to promote behavioral health programs and resolve community health issues.
Key Skills and Qualifications
The ideal candidate will possess excellent written and verbal communication skills, ability to work with diverse populations, and proficiency in MS Office products. Strong communication and leadership skills are required, along with a commitment to building a capable finance team.
