About the Role

We are seeking a highly skilled and dedicated Care Coordinator to join our team of professionals in delivering exceptional care services.

About the Team

The Community Rehab Beds Team provides multidisciplinary assessment and rehabilitation for adults across the Falkirk council area.

Our team is comprised of Physiotherapists, Occupational Therapists, Therapy Assistants, and Rehab Support workers who work collaboratively to deliver Allied Health Professional programmes.

Key Responsibilities
  • To provide high-quality care coordination services to patients and their families.
  • To work closely with the multidisciplinary team to develop and implement effective care plans.
  • To maintain accurate records and communicate effectively with stakeholders.
About You

We are looking for an individual who possesses excellent communication skills, is flexible, and able to work collaboratively as part of a team.

You will have a strong passion for rehabilitation and be committed to delivering exceptional patient care.

Salary and Benefits

The salary for this role is approximately £32,000 per annum, depending on experience.
