About the Role

We are seeking a highly skilled Care Coordinator Lead to join our Early Intervention Team in Oldham. As a key member of the team, you will work closely with clients who have suspected psychosis or are presenting with a psychotic illness. Your primary responsibility will be to undertake formulation, assessments, and plan, monitor, and review care packages.

The successful candidate will have a strong background in mental health nursing, occupational therapy, or social work, with experience in early intervention for psychosis. You will be confident in conducting high-quality risk assessments, implementing risk management plans, and managing Community Treatment Orders.

Key Responsibilities
  • Assess, support, and monitor service users experiencing their first episode of psychosis or at risk of developing psychosis
  • Liaise and co-work with other services, statutory and non-statutory
  • Work with carers and families of service users

You will be based within one of our five Early Intervention Teams across the Pennine Care footprint to the east of Manchester, which has achieved 'top performing' status at the National Clinical Audit of Psychosis (NCAP). The organisation has secured additional investment to expand the psychological therapies service to people aged 14-35 with an At-Risk Mental State (ARMS).

Requirements
  • Registered Mental Health Nurse (RMN / Diploma / Degree), Occupational Therapist, or Social Work registration
  • Qualification to postgraduate level or equivalent work experience relevant to the role

We offer a competitive salary of £40,000 - £50,000 per annum, depending on experience, as well as opportunities for professional development and growth within the NHS.
