Primary Care Liaison Nurse for People with Learning Disabilities

We are seeking an experienced and compassionate Primary Care Liaison Nurse to join our Specialist Health Community Learning Disability Team. In this critical role, you will facilitate open and easy access to hospital and other mainstream health services for people with learning disabilities, ensuring they receive the best possible care and support. Your primary responsibility will be to promote person-centered care, developing effective relationships with healthcare professionals, families, and carers to ensure seamless care pathways.
The salary for this role is approximately £45,000-£60,000 per annum, depending on experience and qualifications. This position offers a unique opportunity to make a real difference in the lives of people with learning disabilities, collaborating with multidisciplinary teams to deliver comprehensive care and support. As a Primary Care Liaison Nurse, you will be part of a dynamic team that values compassion, empathy, and inclusivity. Key Responsibilities:
