Nurse Care Coordinator Job Description

We are seeking a Nurse Care Coordinator to join our team at Newbury Street Practice. As a key member of the healthcare team, you will be responsible for coordinating patient care and ensuring seamless communication between departments.

Key Responsibilities:

  • Co-ordinate patient care and ensure effective communication between departments
  • Develop and implement individualised care plans for patients
  • Collaborate with multidisciplinary teams to achieve optimal patient outcomes

Requirements:

  • Registered Nurse with Nursing and Midwifery Council registration
  • Masters degree required for qualification post December 2020
  • Experience in managing a designated caseload and completing assessments

Salary: £55,000 - £75,000 per annum depending on experience
