About This Role

Key Responsibilities
  • To conduct thorough assessments of young people's mental health requirements, identifying areas for improvement and developing tailored plans.
  • To develop and implement effective care and discharge plans, ensuring seamless transitions between services.
  • To utilize expertise in risk management to adapt changing risk plans, prioritizing the safety and well-being of clients.
  • To work collaboratively with existing systems to ensure efficient completion of discharge planning.
  • To draw on extensive experience working with diverse young people presenting with various wellbeing and mental health challenges.
  • To provide expert referrals to relevant services and make safeguarding referrals when necessary.
  • To leverage expertise in CAMHS and children's child protection work to deliver exceptional support.
Estimated Salary

The estimated salary for this role is £45,000 - £65,000 per year, depending on experience. Our comprehensive benefits package includes access to ongoing training and development opportunities.
