**About the Role**

We are seeking a dedicated and compassionate Care Professional to join our team as a Telecare Response Coordinator.

The role involves providing high-quality support to vulnerable adults across Hertfordshire, responding to calls and visits in a timely manner. The successful candidate will have excellent communication skills and be able to work effectively in a fast-paced environment.

Key Responsibilities:

  • Providing practical and emotional support to Service Users
  • Responding to calls and referrals from Clinicians and Community Health Professionals
  • Assessing service users' needs and creating personalized support plans
  • Supporting service users during hospital stays and after discharge
  • Ensuring seamless coordination with healthcare professionals and community services

About You

  • Full UK manual driving license and access to a vehicle
  • Good knowledge of NHS and Social Care services
  • Ability to make things great by improving service quality
  • Professionalism and diplomacy in dealing with queries and sensitive situations
  • Flexibility to adapt to changing priorities and work patterns
