  • Provide skilled nursing care to patients in their own homes, working independently and as part of a team.
  • Assess patients' needs and develop care plans in collaboration with the multidisciplinary team.
  • Work effectively with patients, carers, and other healthcare professionals to deliver high-quality care.
  • Organise and manage your own time, delegating work appropriately and prioritising tasks to meet service needs.
  • Ensure accurate and timely written records are kept, complying with Trust policy and NMC guidance.
