About the Role

We are seeking an experienced Clinical Community Nurse Leader to join our Unplanned Care Team. As a key member of the team, you will be responsible for leading and managing a dynamic community nursing team, providing high-quality patient care and ensuring seamless transitions between care settings.

Key Responsibilities
  • Lead and manage a community nursing team, promoting excellent clinical practice and patient outcomes
  • Collaborate with multidisciplinary teams to deliver effective patient care and support
  • Develop and implement innovative solutions to improve patient experience and service delivery
Requirements

To succeed in this role, you will need:

  • NMC registration as a Registered General Nurse (RGN) or equivalent qualification
  • Proven experience in community health care, preferably in a leadership or management capacity
  • Excellent clinical skills, with a focus on community nursing and patient-centered care
