About the RoleWe are seeking an experienced Clinical Community Nurse Leader to join our Unplanned Care Team. As a key member of the team, you will be responsible for leading and managing a dynamic community nursing team, providing high-quality patient care and ensuring seamless transitions between care settings.
Key Responsibilities- Lead and manage a community nursing team, promoting excellent clinical practice and patient outcomes
- Collaborate with multidisciplinary teams to deliver effective patient care and support
- Develop and implement innovative solutions to improve patient experience and service delivery
To succeed in this role, you will need:
- NMC registration as a Registered General Nurse (RGN) or equivalent qualification
- Proven experience in community health care, preferably in a leadership or management capacity
- Excellent clinical skills, with a focus on community nursing and patient-centered care
