About the Role

We are currently recruiting for a Substance Misuse Care Coordinator to join our team at Service Care Solutions. This is a fantastic opportunity to work with a reputable non-profit provider who is dedicated to supporting individuals struggling with drug and alcohol addiction.

As a Substance Misuse Care Coordinator, you will be responsible for supporting individuals from point of entry, providing screening, assessment, and recovery plans, as well as delivering one-to-one interventions. You will also be responsible for managing a caseload of 40-60 service users, key working, and care planning.

Key Responsibilities:
* Manage a caseload of 40-60 service users
* Provide screening, assessment, and recovery plans
* Deliver one-to-one interventions
* Key Working and Care Planning

What We Offer:
* Competitive salary of £28000 - £38000 per annum, depending on experience
