Care Coordination Opportunity
We are looking for a skilled Senior Care Assistant to join our team at Ditton Priors Care Centre. As a key member of our care coordination team, you will play a vital role in delivering exceptional care services to our residents.
Your primary responsibilities will include leading and developing a team of care workers, coordinating care planning, medication management, and delivery for designated groups of service users, and ensuring resident participation in care planning and reviewing processes.
Main Responsibilities
- Develop and implement effective care plans that meet the unique needs of each resident.
- Collaborate with healthcare professionals to ensure seamless care coordination.
- Lead and develop a team of care workers to deliver outstanding care services.
- Coordinate care planning, medication management, and delivery for designated groups of service users.
- Monitor and evaluate the effectiveness of care services.
Requirements
- NVQ 3 / MA L3 in Health and Social Care or working towards it.
- Previous experience as a senior in residential care or nursing homes.
- Passion for caring for the elderly.
- Experience with medication administration.
