Practice Nurse

Job Summary
We are seeking an experienced practice nurse to join our team at Primary Care Careers. The successful candidate will have a relevant nursing degree, current NMC registration, and experience in treatment room consultation, phlebotomy, and ECG recording.
Main Duties
- Undertake health assessments, administer medication and treatments, and perform diagnostic tests.
- Provide and evaluate health interventions to ensure patients' health and social care needs are met.
- General health and well-being promotion, creating individual plans for patients with long-term conditions.
- Manage and treat long-term conditions, adhering to national and local policies.
- Review medication for therapeutic effectiveness, supporting patients' adherence to prescribed treatments.
- Prioritize health problems, intervening in complex, urgent, or emergency situations.
- Support women's health needs, including cervical cytology consultations and vaccinations.
- Implement vaccination and immunization programs for adults and children.
- Meet patients' needs for opportunistic wound care, hypertension review, and asthma, COPD, and diabetes clinics.
