Job Description:
Buckinghamshire Healthcare NHS Trust is seeking a skilled and compassionate Patient Care Coordinator to join their team. In this role, you will play a vital part in ensuring the smooth transition of patients with complex discharge needs from hospital to home.

About the Role:
This is an exciting opportunity for a motivated individual to work within our Discharge Team, supporting patients with complex needs and advocating on their behalf when necessary. You will be responsible for liaising with ward staff, community services, and voluntary agencies to develop personalized care plans that meet each patient's unique requirements.

Main Responsibilities:
  • To support the delivery of high-quality discharge and patient flow for patients with complex discharge planning needs.
  • To identify patients with complex and multi-discharge planning needs that require intervention and work with the complex discharge coordinators to ensure all appropriate assessments/referrals are made.
  • To communicate and liaison with patients, relatives, carers, and staff in community, social services, and voluntary agencies to develop an optimum plan for the patient from the point of admission to discharge.

Requirements:
The successful candidate will possess excellent communication skills, both written and verbal, as well as the ability to work effectively in a fast-paced environment. A strong understanding of the NHS and experience working with highly confidential data are also essential. Additionally, the ability to travel to various BHT sites as required is a must.

What We Offer:
In return for your hard work and dedication, we offer a competitive salary of approximately £25,000-£30,000 per annum, depending on experience, as well as opportunities for professional development and growth within the organization.
