About the Role:
As a Care Coordinator at Riverside, you will play a vital role in supporting vulnerable individuals to achieve their full potential. Working in a fast-paced environment, you will provide structured support and risk management to customers with complex needs. Your empathetic nature and can-do attitude will enable you to build strong relationships with customers, helping them overcome barriers and develop life skills.

Your Responsibilities:
  • Lead on co-producing bespoke support and move-on plans for customers
  • Engage customers in regular planned reviews of support and risk plans
  • Assist customers with day-to-day support and tenancy-related matters
  • Identify and promote opportunities for employment, education, and training

  • Competitive salary: £25,965.41 per annum (£27,112.48 per annum after 18 months successful performance)
